The San Francisco Traumatic Brain Injury Network (SF TBI NET) is a collaborative project between the Janet Pomeroy Center, the Department of Mental Health and St. Mary's Hospital. The SF TBI Net began in January 2002 and serves adults with traumatic brain injuries and their families. Traumatic brain injury is defined as an injury that is sustained after birth from an external force to the brain, resulting in cognitive, psychological, neurological, and/or anatomical changes in the brain functions. Our primary goal is to empower adults with a TBI to utilize existing programs to return to the community and achieve optimal independence.
The SF TBI NET provides:
Service coordination to adult TBI survivors who reside in San Francisco county. Service Coordination /includes, but is not limited to, community reintegration, vocational support (see TBI Case Services), supported living services, care giver support and family/community education. All participants receive an initial functional needs assessment from which an Individual Service Plan (ISP) is developed to assist the participant with achieving their goals. Assessments are repeated every six months with ISPs developed as needed. Entrance and exit is flexible with this program, allowing the participant to return for further assistance as needed. Staff provides follow-up on participants for up to one year.
The SF TBI NET also provides information about and referral to resources such as community agencies serving brain injured individuals, financial resources, rehabilitation needs, legal services and care facilities. We also provide community education to the public about TBI. These services are available to the greater Bay Area.
